In consideration, being allowed to participate any way at the Dells International, Centro de Alto Rendimiento, Raifa, Elver park, warner park, soccer leagues, try-outs, clinics, camps, tournaments, any other sports program, events, and activities, the undersigned acknowledges, appreciates, and agrees that:

1. The risk of injury activities involved in this program are substantial including the potential for permanent paralysis and death; however, particular rules, equipment, and person discipline may reduce the risk of serious injury does exist.
2. I willingly agree to comply with the stated and customary, terms and conditions for participation, nevertheless if I observe any unusual substantial hazard during my presence or participation, I will eradicate myself from participation and bring such to attention of the nearest official immediately.
3. I am knowing and freely assume all such risk. Both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or other, I assume full responsibility for my participation.
4. I for myself and on behalf of my personal representative, hereby release and hold harmless, Dells International LLC, Centro de Alto Rendimiento, Raifa, Elver park, Warner park, any public facility that we may use in Madison, Dells international facilities, officers, officials, agents, and employees. Other participants, sponsoring agencies, sponsors, advertisers, referees, and if applicable, owners and premises used to conduct the event.
5. Releases with respect to any injury, disability, death, loss or damage to person or property. Weather, arising from negligence, or release’s or otherwise.

You have read this release of liability and assumption of a risk agreement, fully understand the terms and sign it freely and voluntarily without inducement.

IMPORTANT: You must read all the disclosures, rules, waivers and Dells International policies, the participant/player must be +18 years old. If you’re under 18 years old, the registration MUST BE SIGNED BY PARENT OR GUARDIAN.
